Pre-Authorization Form To have you account paid automatically each month by either your bank account or credit card, please complete this form. Other Information All applications forms are considered complete with 2 pieces of ID, Pre- Authorization Payment information and a requested User ID and Password.

Completed applications can be faxed to 519-272-0037 or 1-866-595-4143 or dropped off at our offices at 140 Downie Street (enter from St. Patrick Street) in Stratford or 16 Mill St. E. in Milverton.

Payment of account is due by the 26th of each month or the next business day. Payment can be made by cash, cheque, credit card, pre-authorized payment through a bank account or credit card, Interac or online.
